Preparing for your First Visit - Important Information Required
Prior to the Appointment, we need you to provide your personal, medical, and dental history, and a release for any records/radiographs from your previous dental office. Please complete at least 24 hrs prior to your appointment so we can review and prepare. This allows the best use your time with us and enables our providers to concentrate on the critical information.
All forms are available online. Links will open another page at a site called Jotform. This is secure and not malicious (HIPPA compliant). If you cannot complete online, you may print forms or call office for copies.

Drop forms at office. Note that it is best if you do this prior to the appointment so we have time to review, obtain any required records from your dentist/medical team. This will make the most of your time with us. Please contact the office if you have questions.

Clinic Location
The clinic is located in the Annex of the Harness Factory building. Just pull into the parking lot on the north side of the Harness Factory building and the clinic entrance is at the back of the parking lot.
Key landmarks are the No Frills parking. We are directly across from them.
